Please see the job description and person specification for further details. To work as part of the prescribing team providing support for the PCN team (both clinical and administrative). To provide medication review services to patients via clinics in the practice. Help support the practices with recalls. To support the achievement of the practices prescribing Quality and Outcomes Framework targets. To assist the clinical and management team with data analysis of prescribing behaviour in line with NICE guidance and local clinical commissioning directives. Improving repeat prescribing processes, including promotion of repeat dispensing, online ordering, minimising clinical risk and aiming to reduce waste in medicines. To support the Prescribing team by sharing appropriate medication guidance and establishing a method of communication to both administrative and clinical staff. Assist in the development of patient information leaflets. To assist with the development and review of medicines audits. To maintain and develop medicines communication to staff. To support staff in the promotion of healthy lifestyles and support those with chronic diseases to develop good lifestyle changes to assist in the management of their conditions. To assist in the development of relationships with secondary care, community specialists and community pharmacists. To support medication changes from secondary care or community specialists in line with the practice policies. Actioning any medication changes that are required after reviewing hospital discharge and clinic letters. Provide a contact for patients, secondary care and community pharmacists with medication queries. Lead on project work/prescribing initiatives/clinical research under the support of the Clinical Pharmacist or Clinical Lead. Help run and keep up to date with the relevant CQC searches within the Network.
