The Nook is a collaboration project between Cheshire and Wirral Partnership NHS Foundation Trust (CWP) and Cheshire West and Chester Local Authority (CWAC). As a member of the Nook team you will work closely with partner agencies and be involved in providing an intensive whole family approach supporting those children and young people who are experiencing mental health difficulties which place them at risk of entering a Tier 4 in-patient unit or where young people are being discharged from a Tier 4 in-patient unit and there may be a risk of family breakdown or readmittance to hospital. The Nook team work with the family as a whole, utilising a solution focused methodology and evidence-based interventions to support and empower them in developing and implementing sustainable, long term, solutions and feel confident in managing the difficulties they may face. As a practitioner you will receive regular supervision by an experienced Senior Practice Lead or Team Manager to ensure the assessed needs of the child are being met and outcomes are improved. As a practitioner, excellent training opportunities will be provided to support you in undertaking the role. Caseloads will be reflective of the need for intensive intervention with the families which will be regularly reviewed and measured against a clear SMART planning process to ensure effectiveness of intervention.
