Our Children’s Asthma and Wheeze service has a great opportunity for you to become part of our specialist children’s community asthma and wheeze service. Previous experience and knowledge of managing asthma and wheeze is preferable, however support and training will be provided on commencement of the role.
The children’s community asthma and wheeze service provide specialist nursing provision for children and young people aged 2 to 16 years with asthma and wheeze in the community who have uncontrolled asthma.
The specialist Asthma and wheeze nurse will undertake assessments and provide support to children, young people and their families to safely and effectively manage their asthma to reduce ill health and hospital admissions. This is achieved by running nurse led asthma clinics and telephone reviews as well as undertaking home and school visits.
The specialist asthma nurses work collaboratively with the specialist respiratory hospital team, GP’s, practice nurses and pharmacists to achieve a high level of support for children and young people with asthma and wheeze. They will also provide asthma training for primary care and education staff across West Hertfordshire to promote safe and effective management of asthma in the community.
You must hold a full valid driving licence and have access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

To work autonomously to provide high standards of assessment and provision of care to children and young families referred with asthma and wheeze in line with the national guidance.
• To manage a caseload, prioritise and delegate.
• Demonstrate a professional approach to work, and act in accordance with the NMC Code of Professional Conduct at all time.
• To act on own initiative and in such a manner as to upload and enhance the good standard and reputation of the Nursing, Midwifery Council.
• To undertake specific delegated responsibility for the teams in the absence of the Team Leader.
• To function as an expert clinical practitioner within the Children’s Asthma and Wheeze Team. To deliver direct patient care whilst, managing, leading, co-ordinating and overseeing nursing practice.
• To ensure that there is efficient and effective utilisation of all resources required to deliver optimum quality care to children, young people and their families who access the Children’s Asthma and Wheeze Nursing Service.

• To work closely with the GP’s and respiratory consultants to develop effective pathways of care and clinical management plans for patients.
• To maintain skills and training for procedures.
• Maintain an update log of all training undertaken.
• Identify and respond to issues of clinical risk.
• Provide information and training to parents based on assessment of need.
• Participate and support regular educational audit demonstrating example on an annual basis.
• To assist in the education and practical development training of all learners, utilising every opportunity for clinical teaching.
• To develop protocols and guidelines to ensure patient centred, age appropriate safe management, for children and young people with asthma. This will include the transition to adult services.
• To recognise own professional limitations
• Demonstrates the ability to manage time and resources effectively.
• Demonstrate an ability to prioritise workload and is able to show flexibility in response to urgent and/or changing needs.
• Through reflection and clinical support, develop strategies to improve, and where necessary change practice.
• Identify issues relevant to practice governance and participate in sharing knowledge in this forum.
• Identify areas of practice development and demonstrate the implementation of one practice development initiative per year.
In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum payment of £1,066 and a maximum payment of £1,845 per annum (pro rata where applicable).
