Client Services Coordinator (CSC)
This position is a full-time position starting immediately. The position is for 5 days per week (@ 37.5 hours/week) based out of our Peterborough office. Regular travel throughout Peterborough, City of Kawartha Lakes, Haliburton and Northumberland Counties. Client service will consist of in-community, office and as needed virtual support (which may include working from home at times). Summary:
The Brain Injury Association serving the four counties of Peterborough, Kawartha Lakes, Haliburton and Northumberland is a unique supportive organization structured as an Association including individuals with the lived experiences of acquired brain injuries, their care partners and both paid and unpaid staff and a Board of Directors.

Client Service Coordinator (CSC) is a position that has been developed with the intent to ensure that individuals with the lived experience of both non-traumatic and traumatic acquired brain injuries are assisted to reach goals and activities that they hold to be meaningful and life-enhancing.
The CSC is responsible for the support of individuals living with the effects of acquired brain injuries through the provision of person-centered service in the communities where they live. The CSC carries out the duties within program teams of the Association. This includes responsibility for providing individualized support and intervention to individuals with ABI in the community through assessment, ongoing individual service planning and evaluation, consultation with other service providers outside of the Association and crisis intervention/referrals.

Job Duties:
* Respect and protect the rights of clients, including but not limited to their right to independence, autonomy, and self-determination; their right to choose and practice individual values, beliefs, religion, and culture; their right to privacy and dignity; and their right to live free from retaliation, discrimination, and abuse.
* Providing individualized support and intervention to individuals with ABI in the community through assessment, ongoing individual service planning and evaluation, consultation with other service providers outside of the Association and crisis intervention/referrals.
* Work with the CSC Supervisor and/or team lead to oversee and assist with the delivery and implementation of program.
* Works with individuals to define professional role, boundaries and development of a strong therapeutic relationship with the client.

* Identify individual needs and assist in the development of personal plans.

* Advocate and assist at the person’s request for needed services and supports from other service providers.
* Provision of crisis resources and development of crisis support plans with other agents.
* Track, review, and evaluate programs that may assist the individuals to achieve their personal plans.
* Ensure that all staff of the Association team is aware of potential issues and crisis.
* Clear, concise and timely client documentation, prepare reports, maintain comprehensive statistics as required.
* Attend weekly staff meetings, virtually or in person as required.
* Attend CSC/CSW team meetings.
* Advocate and raise awareness of brain injury with community partners.
* Reports weekly to CSC Supervisor or team lead on support of assigned clients.
* Participate in ongoing professional development and mandatory training.
* Other duties as required.
Qualifications Required:
* Completion of post-secondary education in the social or health science disciplines.
* Minimum of three years related experience in the delivery of community supports to clients.
* Possess a valid Ontario Driver’s License, automobile in good repair and insurance as required by agency policy.
* Demonstrated knowledge and understanding of psycho-social-emotional-spiritual-rehabilitative principles.
* Knowledge of the local community resources.
* Demonstrated ability to work in clients’ living and social environments.
* Strong communication and interpersonal skills.
* Flexible with demonstrated ability to work independently, as well as with a team.
* Demonstrated knowledge of computer applications (Microsoft Office Suite including Teams and other virtual platforms).
* Satisfactory police record check and vulnerable sector screening.
Work Conditions:

This is a full-time position and rate of pay depends on experience. Wage enhancement applies to hours worked directly with clients. Address COVER LETTER to BIAPR HR. Preference to be given to those with education and experience in ABI, mental health and addictions, as well as supporting victims and survivors of gender-based violence.
