Job overview
We are excited to announce a new Therapy vacancy (either Physiotherapist or Occupational Therapist) as our Urgent 2-hour Community Response (UCR) service continues to develop and adapt to best meet the needs of our local population. This service is provided within the Hospital at Home Service and is an exciting place to work, with many training opportunities available.
This is a 7-day service and operates from 08:00-20:00, with a base at Ellesmere Port Hospital. The service provides UCR support for the residents of Chester and Cheshire West. You would be expected to cover all aspects of the shift pattern. This is a part-time role, 22 hours a week.
You will be a member of a holistic team consisting of GP's, Advanced Clinical Practitioners, Registered Nurses, Therapists, Therapy Assistants, a Pharmacist, and Clerical Support.
The service provides a 2-hour response to patients experiencing a health crisis, aiming to support their treatment in their own home rather than requiring a hospital admission.
The successful candidate will be enthusiastic, flexible, innovative, and enjoy working as part of a dynamic and proactive team.

Main duties of the job
1. Provide a 2-hour response for patients entering a health crisis.
2. Work as an autonomous practitioner to perform a specialist therapeutic assessment of patients with diverse presentations. Use clinical reasoning to provide a diagnosis, deliver, and adapt an individualized treatment programme within the patient's own home.
3. Work in collaboration with the Community Care Teams, Northwest Ambulance Service, Social Services, Rosscare, and the patient's registered GP, to support the patient at home.
4. Work flexibly in response to service needs, ensuring allocation of an appropriate caseload for the Therapy Assistants.
Person specification
Qualifications
* Diploma/Degree in Physiotherapy/Occupational Therapy
* Evidence of relevant and validated post-graduate course
* HCPC registration
* Member of Chartered Society of Physiotherapy (CSP)/Member of Royal College of Occupational Therapy (RCOT)
Knowledge and Expertise
* Scientific and clinical knowledge of physiotherapy practice in line with evidence-based practice/Knowledge of Occupational Therapy specific theoretical frames of reference in line with evidence-based practice
* Professional guidelines and codes of conduct
* Critical appraisal
* Evidence of CPD and portfolio demonstrating interest in a specialist area
* Commitment to Continuous Professional Development
* Adherence to the Code of Ethics and Professional Standards of CSP and the HPC
Experience
* Minimum of 2 years post-graduate experience, including a broad range of working in relevant fields
* Commitment to training and development
* Reflective practice
* Experience of student clinical supervision
* Experience of day-to-day supervision of Band 5 and/or support staff
