Job summary An exciting opportunity has arisen for an enthusiastic and motivated Advanced Nurse Practitioner to join our vibrant, forward-thinking practice. The successful candidate will be an integral part of the nursing team and will be responsible for participating in the delivery of high-quality patient care. Main duties of the job About the Advanced Nurse Practitioner role A requirement for the role is to be registered with the NMC, be an independent/supplementary nurse prescriber and hold an MSC/Post graduate diploma or equivalent. Too perform as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating direct referrals as appropriate. Key responsibilities Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with undifferentiated and undiagnosed conditions and complex needs. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly. Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national/local protocols and within scope of practice. Work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.
