The post holder will work alongside PCN Pharmacy team members, clinicians and other staff over the practice sites and as part of a multi-disciplinary team. They will take responsibility for an area of work that will contribute to fulfilling the requirements of the Network Contract Directed Enhanced Service (PCN DES). This role is designed to ensure new pathways and/or services are co-ordinated efficiently and effectively, ensuring robust communication with all stakeholders. The post holder will support and co-ordinate group consultations across the PCN. They will also support the Diabetes MDT clinic, and other clinics as required by the primary care network and the practices.
