Within the borough we have award winning parks, open spaces and listed heritage sites. The Brokerage team deliver services in the Community via a hospital discharge or via Strength based panel. We support people into care Home Placements, Supported Living Accommodation, Complex placements for people with an LD and support with set up of Direct Payments which includes supporting with budget and setting up the pre-paid card. About The Role The Brokerage and Commissioning Team supports adults with physical, mental or learning disabilities to access care services either by way of a commissioned service on their behalf, or to support them in managing their own care package through the provision of a personalised budget. The role also includes support with placements and emergency respite, LD placements and Supported Living Services. The Care Broker role will be allocated work to ensure the smooth delivery of these services. The team covers the whole of Bexley and is currently based in one location which acts as a resource base for Social Workers in the Borough, due to ASC pathways changing to 3 LCN’s and will cover the 3 LCN areas and will have good communication skills to support a seamless service for our users. You will work as part of the Brokerage Team, based at Civic Offices Bexleyheath. The role will be to arrange homecare services and assist with placements for people who have an assessed need which include LD, Supported Living Services and Younger people as well as Older people who may have very complex needs. You will support the team to undertake all brokerage tasks in a timely way. You will also be part of a team that supports people who wish to take a direct payment and manage their own services. You will help to ensure that discharges from hospital and the provision of community services are made with accredited providers in a timely manner. Attention to detail is paramount to ensure that each client is cared for appropriately. Multi agency working is an essential part of the role as we work closely with Hospitals, Care Providers, and Social Care staff. Relevant Experience In This Field Is Required.
