About the role
Waythrough's Community Diversion Service is funded by the Northumbria Violence Reduction Unit supporting their vision to ensure Northumbria is a place where individuals, families, and communities thrive, making the most of opportunities to live, learn, and work free from the fear of violence.
Working across the Northumbria Police Force area, this service supports Children and Young People from 10 up to 25 years old (alongside their immediate families) who are vulnerable and/or at risk of being involved in serious violent crime.
Summary of role
Are you passionate about making a positive impact on individuals involved in serious violent crime? We're looking for two dedicated Hospital Link Workers (to work within Northumberland Hospital A&E department) to play a pivotal role in our mission to support service users make positive changes to their life and reduce their likelihood of committing serious violent crime. Humankind’s Hospital Link Workers will deliver support to individuals that present within A and E departments with injuries relating to serious violent crime and support them to access the Community Diversion Service where they will then motivate and encourage the individuals to both make positive changes to their lives and work with specialist services in order to achieve further positive outcomes.
As a Hospital Link Worker, you will be responsible for:
* Caseload Management: Effectively manage a caseload of service users, providing comprehensive support tailored to their unique needs.
* Intervention Planning: Devise, implement, and review ongoing support plans and treatment strategies in collaboration with the service users. Your role is instrumental in helping them achieve their goals.
* Collaborative Approach: Work closely with a diverse team of professionals, including fellow colleagues, family members, and significant others, ensuring a holistic and integrated approach to support.
* Goal Setting: Assist service users in setting and working towards their goals, fostering a safe and supportive environment.
Please note there are two vacancies available: 2x Northumberland (both post holders need to have access to a car and be able to commute to a Newcastle based office when required)
This post is a Hybrid Working with shift patterns (However, there may be a need to work flexibly to account for your service user’s needs):
Week 1: you will work 09:00 am- 17:00 pm Monday-Thursday and 09:00-16:30 on Fridays, 37 hours within the community.
Week 2: you will work 14 hours within the hospital Friday and Saturday 17:00-00:00 and 23 hours within the community (exact community hours will be arranged with your line manager)
About you
To succeed as a Hospital Link Worker at the Community Diversion Service, you will ideally have:
* A strong commitment to the well-being and rehabilitation of service users.
* Substance misuse, support work, mental health, or criminal justice work experience.
* Excellent communication and interpersonal skills.
* Ability to work effectively within a multidisciplinary team.
* Knowledge of best practices in safeguarding children and vulnerable adults.
* Adherence to ethical standards and guidelines in the field.
