We are looking for two experienced senior support workers to join our team at our city-wide, three bed Short Break Service in Wythenshawe, Manchester. The role will involve providing person-centred care and support to individuals with a known learning disability who have associated complex communication and behaviour difficulties. The Short Break Service is led by Learning Disability Nurses, alongside a PBS Practitioner and the team of senior support workers to provide short breaks, working closely with wider community team. We are part of the public sector and partnership organisation powered by Manchester University NHS Foundation Trust, Greater Manchester Mental Health, Manchester City Council, Manchester Health & Care Commissioning and the Manchester Primary Care Partnership Main duties of the job This role involves delivering care and support using person-centred approaches embedded within the PBS Model. Short Break Service is a busy service which supports adults with learning disabilities, who may also have a diagnosis of autism, complex communication and behavioural difficulties. Main duties include; providing care and support to individuals accessing the service including personal care and meals. Supporting with community access using public transport or the services vehicle, health monitoring, medication administration and managing finances. Senior Support Workers have link worker responsibilities and involved in the development of care plans and risk assessments and activity planning. Senior support workers must be over 21 years old and hold a valid UK driving license as the service provides transport using a large (manual) nine-seater vehicle. This role includes link-working alongside the named nurse, liaising with members of the wider MDT and familiesto develop care plans and increase outcomes for this person. Shift patterns include days and nights, covering a 24 hour service, seven days a week inclusive of sleep in and waking nights as per rota. Ability to work unsupervised and without direct managerial input. Lone working may be required.
