Then Cheshire West and Chester Council could be the perfect place for you. We are seeking six new social workers to join our Hospital Discharge team, either on a permanent or temporary basis. The positions are based council wide and if successful you will be working at both hospitals and council offices. These roles cover any Cheshire West resident needing discharge support to leave hospital. However, we work with our team to consider preferred locations to maximise their time and use of skills. You will work hand in hand with medics, therapy, nursing & reablement teams all focused with the same goals of completing personalised holistic assessment to support patients home safely and without delay. As a Social Worker in the team, you would have the following responsibilities; Be person centred and reinforce a patient's choice and control. Promote a patient's well being throughout their journey in the acute and community setting. Provide Strength Based Assessments and independence. Complete Care Act 2014 compliant assessment. Guide and support colleagues less experienced in social care processes. Knowledge related legislation, policy, and frameworks. Experience of Safeguarding processes and procedures. Complete mental capacity assessment and best interest processes for patients unable to make specific decision. Knowledge of the Deprivation of Liberty Safeguards. Attend MDT's, professionals' meetings, discharge planning meetings and best interests' meetings as may be required. You will have: CQSW/CSS/Diploma/Degree in Social Work or equivalent (Essential) Social Work England Registration (Essential) Be required to undertake an enhanced DBS plus Adults Barred List (Essential) Statutory and policy base of Social Work (Essential) Appreciation of recent developments in service provision. Ability to communicate effectively with customers, carers and families. Ability to cope positively with change and an ability to promote positive changes to make a difference. Awareness of importance of multi-disciplinary working and an integrated approach. Ability to carry out assessment and support planning in line with relevant legislation. You are confident, energetic and enthusiastic as well as sensitive and resilient. Self-motivating individual with the passion to make a difference in adult social care.
