We are looking for a Community Support Worker to join the Tiverton Core Adult Mental Health Team based at Tiverton Community Hospital.
The role is dynamic and focused on rehabilitation and recovery. The role will provide direct therapeutic engagement to services users focusing on desired future goals and solution building.

Main duties of the job
1. The support worker will have experience in supporting people with mental health difficulties.
2. The support worker will be able to support service users with emotional, social, physical and domestic needs.
3. The support worker will assist clients with their activities of daily living.
4. The Support Worker will develop strong links with carers and other networks to support the service user to develop a co-produced management plan that allows them to manage their mental health and build on existing skills and community links to further support their future management and wellbeing.

Job responsibilities
1. The support worker will provide direct therapeutic engagement to services users, supporting them in a co-constructed manner, focusing on desired future goals and solution building.
2. As part of the Core Adult Mental Health Team you will develop strong links with carers and other networks to support the service user to develop a co-produced management plan that allows them to manage their mental health and build on existing skills and community links to further support their future management and wellbeing.
3. The support worker will support service users to maintain their activities of daily living.
4. The support worker will be supervised by Senior Mental Health Practitioners and will be closely supported as necessary in the management of risks and any potential decline in a person's mental state by the Core Team.
5. The support worker will have clear pathways of escalation and will work within the bounds of existing DPT policies and procedures as required.
6. The Post Holder will be able to demonstrate excellent communication skills with both services users, carers and organisations. You will be highly motivated and display a good ability to work autonomously where possible.
7. The support workers will work with service users with the full range of mental health diagnoses who find themselves under the care of the Core Adult Mental Health Team who will provide clinical direction and support.
8. The support worker will be proactive in seeking out the positive support networks available in the community for each individual which may help them in their recovery.
9. The support worker will be able to support service users with emotional, social, physical and domestic needs. Skilled in engaging both service users and stakeholders and have a good understanding of the needs and diversity of those you will be supporting. Able to demonstrate the ability to work in a therapeutic and compassionate way.
Person Specification
Qualifications
* NVQ level 3, QCF Level 3 in Health and Social Care or equivalent qualification.
* If equivalent qualification is not already obtained, to complete QCF Level 3 in Health and Social Care within 12 months of commencing employment.
* Other evidence of training or development relevant to working within mental health settings.
Experience
* Previous employment / experience of working with people in a caring situation or previous experience working in a different role within mental health services.
* Previous care experience with people who have Mental Health difficulties.
* Knowledge of mental health and a basic understanding of physical health presentations.
* Awareness of safeguarding principles.
