What You Will Do:

Skills and Qualifications:
1. Essential: Degree in Social Work with current Social Work England registration.
2. Experience: Previous experience working in a hospital discharge team or adult social care setting.
3. Knowledge: Strong understanding of the Care Act 2014, safeguarding procedures, and best practices in hospital discharge.
4. Skills: Excellent communication, analytical, and report-writing skills, with the ability to manage complex caseloads.
