Location: Humanity House, Colliers Way, Phoenix Park, Nottingham, NG8 6AT, with travel around the area.
Contract: Fixed Term Contract Until 31st March 2025
Hours: 35 per week
Salary: £22,625 per annum
Driving: Full UK Driving Licence and access to a vehicle.

A day in the life of a Healthcare Assistant will involve:
Providing invaluable contribution to the Community Crisis Support Service and its users.
Providing short term crisis support to people in their homes, helping to keep them out of hospital.
Assessing clients' needs and develop a care plan to fit them.
Providing support with personal care, medication prompts, end of life care, light meal and drinks preparation and emotional support.

To be a successful Healthcare Assistant, You’ll need:
NVQ Level 2 or equivalent in health and social care
Ability to communicate with health and social care professionals
Ability to deal with service user queries in a diplomatic, professional and confidential manner
Knowledge of supporting service users, who may be disabled and/or frail, cognitively impaired.
Good geographical knowledge of the service catchment area.
IT literate
Knowledge of services provided by the NHS and Social Care
Good working experience of the delivery of social care support including personal care, feeding and medication prompting
Participation in a multi-disciplinary team environment
Full driving licence holder and access to vehicle, ability to use the vehicle on a daily basis for service user visits
Willingness to work flexible hours
