We have an exciting and stimulating opportunity for a band 7 Patient Flow and Discharge Co-Ordinator. This post is a permanent position within Chapel Allerton CSU. Chapel Allerton Hospital (CAH) is part of the Chapel Allerton CSU, which also includes Wharfedale Hospital. CAH is based just outside of Leeds City Centre and is part of the Leeds Teaching Hospitals NHS Trust. Our expanding and developing elective orthopaedic hub is based at CAH and includes 26 inpatient beds and a 4 bedded Enhanced Care Unit. Specialist centres for Complex and Stroke Rehabilitation are also based at CAH providing regional collaborative care for our patients and their families. Dermatology and Rheumatology specialist services are provided at CAH, including our regional Laser Service, dedicated diagnostic and infusion suites, and inpatient beds. The CSU bed base can flex up to 100 inpatient beds depending on service need. We also provide extensive outpatient services. Main duties of the job Work collaboratively with the MDT to undertake risk assessments and prioritise transfer and admission of patients within the clinical service. Ensure effective communication with all members of the MDT to support appropriate flow and placement of patients. This involves highly complex and at times highly sensitive information related to specific cases where situations may be extremely delicate. Has the responsibility to assimilate complex information and situations balancing risks and appraising options to ensure that patients are in the appropriate setting to receive the required care and or intervention. Make clinical assessments and treatment decisions within clearly defined policies and protocols with input from the MDT as appropriate. Anticipate problems and take preventative or mitigating actions, or have contingency plans available. To plan and organise complex activities or programs that require formulation of a plan or adjustment of existing plans where internal processes need to be reviewed. This may include leading on operational activities identified by the Matron and escalating issues or problems as they occur. This will involve initiating, planning and organising work packages at times. Provide specialised advice to patients, families, carers and the multidisciplinary team, contributing to the appropriate placement and care of patients.
