Stockport NHS Foundation Trust are looking for an enthusiastic, innovative Occupational Therapist to join our friendly, forward-thinking team providing rehabilitation for a diverse range of people with stroke and neurological conditions within Stockport.
Based in the community, you will work within a comprehensive multidisciplinary team of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Clinical Psychologists, Nurses, Assistant Practitioner, Therapy Assistants and Admin. You will be welcomed as a valued member of the coordinated and committed team, sharing knowledge, skills and opportunities to learn.
You will be provide specialist knowledge to facilitate the early supported discharge of stroke patients from the stroke unit, and work with patients with acute and long term neurological conditions to provide rehabilitation in the home environment, and other community settings, improving their quality of life.
Support and education is provided with the opportunity to work directly with experienced and specialist clinicians across the team and the chance to contribute meaningfully to a fast-paced and growing team.
Main duties of the job
You will undertake comprehensive assessments and use your advanced clinical reasoning skills to develop and deliver rehabilitation programmes that support patients to achieve their goals.
You will participate actively in our weekly MDT meeting, linking with therapists from other disciplines and helping to ensure fully co-ordinated communication and goal-setting. Within the multidisciplinary team you will provide support and training to our Assistant Practitioners and Therapy Assistants within the delivery of rehabilitation interventions and a supervisor, as well as providing clinical education to student members of the team.
As a member of the CNRS team you will have excellent assessment, communication and problem-solving skills and demonstrate an adaptable, flexible approach whilst meeting guidelines and standards.

Job responsibilities
The post offers an opportunity to gain experience in Stroke Therapy in the community.
The Occupational Therapist will act as an autonomous practitioner both professionally and legally accountable for all aspects of their professional and clinical work
Assess and treat patients promoting timely hospital discharge
You will workas part of an integrated multi-disciplinary therapy team to improve flow and quality of a patients journey.
The caseload will be variable and comprise mainly of patients diagnosed with stroke but will include a variety of conditions.
Optimise the clinical progress and quality of life of patients by providing individualised Occupational Therapeutic assessments, evidence-based recommendations and treatment plans.
Work as a committed member of the Occupational Therapy team, contributing to the achievement of Departmental, Team and Trust goals.
Provide relevant advice and training to health care professionals on speciality.
Provide specialist knowledge across a range of procedures underpinned by National standards and theory.

If successful this vacancy will require an enhanced DBS check at a cost of £41.90 This cost will be deducted from your first Trust salary

Person Specification
Education & Qualifications
* Degree in Occupational Therapy.
* State Registration with the Health Professions Council.
* Approved courses, conferences or study days which support Occupational Therapy knowledge and/or practice within rotational or static speciality area
Knowledge
* Professional knowledge acquired through relevant science-based degree
* Specialist knowledge across a range of Occupational Therapy procedures underpinned by theory
* Awareness of audit and its role in clinical practice
* Awareness of policies and procedures and guidelines
* Awareness of Occupational Therapy equipment / interventions and clinical reason(s) for use.
* Basic understanding of NHS structures and systems
* Awareness of cultural diversity and equality issues
