We are seeking a part time Nursing Associate at Wake Green Surgery, based within the Moseley, Billesley & Yardley Wood Primary Care Network (MOBY PCN).

The post holder is a Nursing Associate registered with the Nursing and Midwifery Council. They are required to act within their professional boundaries, supporting and working alongside a multi-disciplinary team within general practice and a Primary Care Network (PCN).
The post holder will contribute to providing nursing care to patients registered in the GP practice, which sits within MOBY PCN.
Main duties of the job
The position of Primary Care Network Nursing Associate has an essential role in improving the healthcare we provide to our patients.
The post holder will focus on assuming a lead role in specified areas, e.g., vocational and employment advice to patients, activities coordination, assisting in individual and therapeutic group work. Work with the qualified nurses to deliver individual treatment packages with an emphasis on collaborative care that focuses on recovery and the need to work towards discharge where appropriate. The post holder will work with other members of the Multi-disciplinary Team (MDT) to both organise and facilitate care appropriate to the individual needs of the patient.

Job responsibilities
The Nursing Associate will work under the supervision of the Practice Nursing Team within the practice.
The Nursing Associate will work within the community to support the everyday running of the practice/PCN. Nursing Associates are equipped with the knowledge, understanding, skills, attitudes, and behaviours to support general practice. The Nursing Associate will assist with the delivery of health promotion, phlebotomy, chronic disease management, immunisation and vaccinations and other clinical roles in line with their training and level of experience.
The Nursing Associate will have a key role in supporting delivery of the new Network Contract DES Service specifications.
The actual duties of the post holder will be negotiated with the practice/PCN depending on the practices needs, contractual obligations and the Nursing Associates knowledge, expertise and experience.

Qualifications
* Registered Nursing Associate on the NMC register
* GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent
Knowledge & Expertise
* Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
* Understands and acts in line with NMC professional standards for practice contained within The Code.
* Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice
* Understand requirements for NMC professional Revalidation
* Understanding of the importance of following procedures and treatment plans
Skills
* Ability to take part in reflective practice and clinical supervision activities
* Ability to organise and prioritise own delegated workload
* Ability to deal with non-routine and unpredictable nature of workload and individual patient contact
* Ability to communicate effectively (written, verbal and non verbal communication) with patients/relatives and carers and all members of the multi-disciplinary team.
* Ability to develop effective and appropriate relationships with people, their families, carers and colleagues
