* Provide assessment and implementation of evidence-based interventions for service users who are experiencing common, acute mental health and longer-term recovery needs.
* Optimise service users' ability to remain in the community and/or be cared for in the least restrictive environment in line with the principles of recovery, social inclusion, and the 10 essential capabilities.
* Facilitate face-to-face assessments and participate in the provision of a comprehensive triage service.
* Plan interventions, support, and educate service users referred to the service with a range of psychiatric illnesses.
* Maintain responsibility for a caseload of approximately 25-30 service users with severe and enduring mental illness, depending on the role within their treatment.
The post holder will:
* Understand mental health difficulties and how to assess and support patients in implementing collaborative treatment plans towards achieving personal recovery.
* Show compassion and care for others—both those we offer a service to and their carers/supporters, as well as colleagues and other agencies.
* Have the ability to work autonomously and, at times under pressure, closely with other professions as part of multidisciplinary working, utilising skills attached to their professional discipline (nursing or social work).
* Demonstrate good organisational skills and the ability to prioritise clinical tasks in order to meet competing demands.
