Main duties of the job
Our Reablement Support Workers provide vital assistance to individuals in their homes post-hospital discharge. They play a pivotal role in helping customers achieve their goals, working closely with dedicated occupational therapists to co-develop personalised reablement plans.
We are seeking individuals with a genuine passion for helping others who are available on a rota basis between 7am to 10pm, 365 days a year. Join our team and receive a £650 welcome payment which you will receive upon starting and a further £650 upon successful completion of your probation (subject to tax and NI).

Job responsibilities
Under the guidance and supervision of the Senior Reablement Support Worker (and from time to time other members of the reablement service management team), your role will be to assist customers to achieve the goals and objectives set out in their person-centred reablement support plan. This will include:
1. Providing customers with personal care and facilitating customers with daily living activities.
2. Work with customers to help them regain their independence with daily living skills, including washing, dressing, meal preparation, and personal care.
3. Safeguard the health, safety, and wellbeing of customers as well as colleagues.
4. Take part in staff development programmes, assuming responsibility for own professional development, i.e., undertake all mandatory training relevant to the service and part of the training pathway for the role e.g., Care Certificate and specialist reablement training.
5. Assist customers with the use of mobility aids and assistive equipment where required.
6. Deliver reablement support in a range of settings, primarily customers' own homes but, if required, also day centres, day hospitals, and in the community.
7. Assist customers with moving and positioning, adhering to health & safety requirements as well as organisational policy and procedures.
8. Contribute to daily communications, accurate record keeping, recording actions, interventions, and incidents in customers' records.
9. Participate in regular team meetings and customer review meetings.
10. Ensure compliance with the Data Protection Act and maintain customer confidentiality and security of information at all times.
11. Communicate effectively with the immediate team and other health and social care professionals as appropriate.
12. Participate in own supervisions, appraisals, and objective setting.
13. Assist the team with general duties if required e.g., office filing, answering telephone calls etc.
14. Work as part of the team, offering a flexible approach to staff rostering.
15. Abide by the principles and practice set out in the staff handbook.
Specific requirements for this post:
1. To work shifts to meet the needs of the service including early morning, evenings, weekends, and bank holidays. An unsocial hours payment for unsocial hours working will be paid where appropriate.
2. To undertake travel across the Wiltshire area to carry out customer visits.
Person Specification
Experience
* Experience which demonstrates the ability to undertake the role.
* An understanding of relevant procedures and working practices.
* Awareness and understanding of the client group.
* Training and knowledge in a range of procedures and medical equipment to support clients.
* Basic knowledge of Wiltshire Council professional groups and external agencies, relevant to the role.
* Sensitivity and empathy to build trusting and supportive relationships with clients and families.
* Care skills and basic knowledge of mobility, nutrition, and financial issues in order to provide practical, emotional support to clients and families.
* Ability to follow routines, carry out set care plans, record and monitor changes in client condition.
* Basic literacy and numeracy.
* Ability to communicate one-to-one and in small groups with relevant client group.
Qualifications
* Care Certificate or equivalent skills and experience with willingness to undertake the required training.
* General level of education to GCSE A*-C or equivalent skills and experience.
* Empathy with the elderly and/or people with a disability and an understanding of their needs.
* Ability to work alone and as part of a team.
* Ability to prioritise and organise own workload.
* A pleasant and helpful manner.
* Effective communication both written and oral.
* Previous experience of providing care and support to the elderly and/or people with a learning disability.
* Basic knowledge of safeguarding.
* Basic knowledge of regulations relevant to community care.
