Main duties of the job You will be responsible for managing a caseload of eligible children and young people, reviewing packages of care and support. You will act as a key point of contact for children, young people and their families. You will work with the child or young person and their families and carers to produce a personal health and support plan, working with system partners to ensure the delivery of holistic packages of care and to ensuring identified care and support needs are met. You will work alongside Adult services to ensure transition to adulthood is smooth. You will take part in the triage of new referrals and undertake assessments of eligibility for Children and Young Peoples Continuing Care with a multi disciplinary team, presenting recommendations to panel. As part of the role you will also be responsible for driving forward designated key strategic areas in relation to the delivery of Children and Young Peoples Continuing Care in Dorset.
