The Community Mental Health Teams (CMHTs) provide care and treatment to a defined group of service users care coordinated under Part 2 of the Mental Health (Wales) Measure 2010. We are pleased to offer the chance for 2 highly motivated and compassionate mental health nurses to join the teams as co-occurring practitioners.
These are new roles, and the successful candidates will play a pivotal role in developing the roles alongside the senior nurse and taking forward evidence-based care for those service users who experience complex mental health and substance misuse needs within the CMHTs. Additionally, the role will play a key part in driving forward the Welsh Government Co-Occurring Agenda within Mental Health and Substance Misuse Services.
We are looking for practitioners with a real knowledge and passion to improve outcomes for this service user group and with a proven ability and commitment to ongoing education and support to the mental health services around co-occurring issues.
Main duties of the job
The post holder's role will include:
1. Setting and maintaining clinical standards within the governance framework and motivating and leading team members from both Community Mental Health Services and Substance Misuse Service (SMS) services to reach specific clinical, health, and Co-Occurring framework targets.
2. Developing good relationships between services, both statutory and third sector, to promote the wellbeing of individual service users and their families.
3. Allocating work and providing professional support to Barod workers within the service.
4. Conducting and coordinating comprehensive assessments of need to formulate effective care plans for the benefit of service users and their families, as well as agreed service standards and outcome measures.
5. Providing clinical teaching and education to other professionals to enhance clinical practice and improve care provision.
6. Contributing to service-based audit, research, and evaluation to provide evidence-based service improvement to the standards of care received by service users.

Essential
* Qualified Health Professional with an RN (Mental Health) qualification
* Professional degree
* Management/leadership skill and evidence of being able to work autonomously
Desirable
* Enhanced therapeutic training, e.g., CBT, MI
* Leadership qualification/leadership specific training
Essential
* Extensive knowledge of Co-occurring Mental health and substance misuse Issues and ability to confidently articulate these to a wide and diverse range of staff
* Enhanced knowledge of the Mental Health (Wales) Measure 2010 and Welsh Government Co-Occurring Framework (2016)
* Excellent risk assessment skills
* Significant experience in the areas of community mental health and/or community substance misuse
