Our Hillingdon Mental Health Floating Support Team provides housing-related support to adults with mental health issues living in Hillingdon. Support is delivered in a variety of settings including service users’ homes, community settings and where relevant, in the service user’s workplace or learning place. Being a support worker is a very rewarding job. About the role: you will, deliver housing related support to customers in their own homes and in the community, including support with developing independent living skills, benefit maximisation, accessing and attending health appointments, accessing and engaging with social and learning activities, volunteering and community engagement. support with finding and securing alternative accommodation. support customers in setting up and maintaining a home / tenancy, including support with maintenance, repairs, payment of rent and arrears, and being a good neighbour. You will work with 13 customers using a strength-based, person-centred approach. We support our customers using the recovery model and co-produce SMART support plans with them to enable them to achieve their goals, sustain independence and enjoy a good quality of life. You will be responsible for ensuring support plans and risk assessments are in date. minimise unplanned and emergency hospital admissions for mental health relapse by offering general support, including emotional support and advice, support with monitoring own health and well-being and signposting to health services. The service operates Monday – Friday 9am to 5pm, and to be successful you will need your own transport and driving license. Benefits you will receive with this role.
