Main duties of the job To work as an integral part of the Young Adult Team: assessing, implementing and evaluating nursing care for service users living with palliative or complex health conditions. To complete a wide range of clinical interventions To act as a mentor / role model to Care Support Workers To support young adults in all aspects of living their lives, including support with activities of daily living, attending appointments and providing a range of social and leisure opportunities.
