Primary Responsibilities: Document contemporaneous consultations in accordance with NMC guidelines using the S1 clinical system. Maintain accurate clinical records in compliance with legislation. Stay informed about statutory and local clinical protection procedures, including referral systems. Provide routine nursing care to patients based on clinical evidence. Perform venepuncture following local guidelines. Request pathology services as needed and process results if required. Adopt a holistic approach to wound management, implementing care in line with current evidence-based guidelines and local protocols. Remove sutures and perform ECGs. Obtain and document informed consent, whether verbal or written. Adhere to infection control guidelines. Preserve the confidentiality of information gained at work at all times. Use read codes and templates effectively. Identify, manage, and support patients at risk of developing long-term conditions to prevent adverse health effects. Conduct chronic disease clinics, providing necessary patient care and referring to secondary or specialist care as needed. Maintain chronic disease registers. Offer advice on childhood and adult vaccinations, ensuring vaccines are administered under patient group directions. Provide vaccination and travel medicine services. Prioritise health issues and intervene appropriately. Support the clinical team with safeguarding matters in line with local and national policies. Deliver opportunistic health promotion when appropriate. Perform holistic assessments for patients attending cervical cytology smear tests (if providing this service). Recognise, assess, and refer patients with mental health needs. Be aware of identification and reporting procedures related to professional standards. Recognise and manage anaphylaxis according to current UK guidelines. Perform cardio-pulmonary resuscitation in line with current UK guidelines. Secondary Responsibilities: Develop, implement, and embed health promotion and well-being programmes. Implement and evaluate individual treatment plans for patients with chronic diseases. Assist GPs with minor surgery when required. Chaperone patients as necessary. Support the team in managing clinical emergencies. Liaise with external services/agencies to ensure appropriate patient support. Monitor and ensure the safe storage, rotation, and disposal of medicines. Support junior nursing team members, providing guidance when needed. Participate in local initiatives to enhance service delivery and patient care. Support and engage in shared learning within the practice. Continually review clinical practices, responding to national policies and initiatives as appropriate. Participate in the review of learning events. Educational Responsibilities: Identify personal development and training needs in collaboration with the GP Nurse Lead and Practice Manager. Participate in clinical supervision and annual appraisals. Support others with their training and development needs.
