This role is part of Bedford's community mental health transformation programme. The successful candidate will work within Primary Care Networks alongside GPs, practice nurses, and Additional Roles Reimbursement Scheme (ARRS) workers (e.g., social prescribers, health coaches and care coordinators) as part of a multidisciplinary team (MDT). PCNs serve populations of approximately 50,000 registered patients
Key Responsibilities:
• Collaborate with GPs, social care, specialist mental health services and voluntary organizations.
• Conduct triage, assessments and deliver tailored interventions to individuals with mental health needs.
• Facilitate access to mental well-being services and coordinate care across agencies.
• Adopt a holistic, empathetic approach to support service users as individuals beyond their mental health diagnosis.

Main duties of the job
• To undertake triage and assessment within the primary care setting and deliver brief intervention or facilitate access to support within voluntary care sector, core mental health or specialist services.
• To work both with individuals that are presenting with a first presentation of mental health need and those with on-going mental health needs that no longer require support from the core mental health team.
• Work as an autonomous practitioner to deliver a range of mental health interventions with support from the senior mental health practitioner and the wider allied core mental health team as required.
• As a member of the multi-disciplinary primary care team, promote awareness and understanding of mental health and wellbeing and provide support to primary care colleagues in recognising mental health problems and formulation of personalised and holistic support plans.
