We have an exciting opportunity for a Rehabilitation Assistant in the Wigan Borough Community Division. This is a permanent full-time post (37.5 hours) to be working across Monday to Sunday. The post holder is required to work without supervision, undertaking treatments and decision making within agreed protocols, underpinned by appropriate experience/knowledge.
To promote and sustain independence/wellbeing for the individual in the relevant community setting through a holistic approach that encompasses patient values and beliefs.
To independently carry out agreed treatment plans set by an Allied Health Professional/Qualified Clinician. This will be conducted using a competency-based level of practice to complete subjective, objective, and analytical components of the delivery of rehabilitation. There will be an integral part of the Intermediate Care Team and will be expected to discuss the progression of future potential changes to patients' treatment plans with other members of the team.
To work independently, without direct clinical guidance, monitoring response to intervention and providing feedback to the qualified clinician in a timely manner when needed.
To support the Integrated Community Service model within Wigan Borough.
We are aiming to recruit suitably qualified, enthusiastic, and engaged individuals to our new Intermediate Care at Home Team. The Team will provide rehabilitation for patients in their own place of residence and other community settings. Joining up the services provided by the 'Home first' Team in addition to Community Therapy, whilst working closely and flexibly with social care colleagues and the existing Intermediate Care Therapy Team who deliver rehabilitation into our existing bedded units.
The ideal candidate will have an open-minded and flexible approach, a 'can do' attitude, and be willing to come into a new initiative to support the development and establishment of the service. They should have excellent communication skills, function as an effective member of the multi-disciplinary team and the integrated team within Health and Social Care, and have the ability to comfortably work across role boundaries.
It is essential for this role that you have the ability to independently travel across the borough to assess, formulate therapy plans, and deliver therapy interventions for patients in their own place of residence.
UK NHS experience is essential, along with Level 2 in Health & Social Care.
