An exciting development opportunity has arisen within our Hartlepool Elective HUB for an autonomous Nurse Practitioner who is dynamic and innovative to join our team of Practitioners within this Nurse Led site.
There is the opportunity to develop further to extend your skills and knowledge and train to become an Advanced Clinical Practitioner within this field.
You will be expected to rotate short periods cross site to ensure yourself and team maintain their acute skills and knowledge.
Our Hub is the first stand-alone Nurse led unit in the country and an excellent opportunity for the right person who is committed and passionate in promoting and driving services forward. There are lots of opportunities for development and training.
The candidate should be a professionally credible first level registered nurse with substantial experience of Elective Orthopaedic and Surgical Care. A well-respected professional and experienced clinical nurse who sets high standards and leads by example to motivate and empower others.
Main Duties and Responsibilities:
1. Deliver and participate in the clinical care of patients in the elective care Unit at UHH and any deteriorating patient on UHH site.
2. Prioritise health needs and intervene appropriately.
3. Recognise any deteriorating patients and escalate accordingly.
4. Transfer unstable patients across site or to another centre accordingly.
5. Be professional and legally accountable for all work undertaken to practice at an advanced level of professional autonomy and accountability that is within Trust policies and the Nursing and Midwifery Code of Conduct (2008) or the Health & Care Professions Council (HCPC).
6. Develop a level of advanced and comprehensive health assessment skills based on a specialist body of knowledge under supervision.
7. Assist in appropriately assessing, examining, investigating, diagnosing and treating patients, resulting in the safe management and appropriate referral or discharge of patients with undifferentiated and undiagnosed presentations.
8. Assist with the management of patients within ward setting.
9. Develop the ability to identify the need for appropriate diagnostic tests and interpret the results.
10. Identify and intervene proactively where circumstances contribute to an unsafe environment for patients, staff or relatives and escalate these to the Care Group/Departmental Risk Lead and/or Consultant Leads for Clinical Governance.
11. Work within the multidisciplinary team, with agreed medical, nursing and Trust protocols and guidelines.
12. Manage own workload.
13. Work as a member of the multidisciplinary team sharing in the responsibility for the provision of 7 days, 24-hour clinical care.
14. Promote and adhere to the principles and practice of Clinical Governance.
15. Demonstrate safe use of equipment and efficient use of resources.
16. Utilise a developed knowledge of child protection and vulnerable adult safeguarding procedures.
17. Assist in the development and implementation of Elective Care pathways and the expansion of these across the Trust working with individual specialist’s areas.
18. Contribute to clinical risk management.
19. Complete documentation and gather evidence relevant to clinical risk management.
20. Anticipate, recognise and prevent situations that may be detrimental to self and the health and wellbeing of patients or staff.
21. Adhere to and participate in the implementation of recommendations and guidelines related to Health and Safety at Work, Control of Substances Hazardous to Health Legislation and Control of Infection procedures.
22. Implement and lead any actions to be taken in the event of an emergency with support from the Duty Clinical Practitioner.
23. Demonstrate correct moving and handling procedures following Trust protocols and guidelines.
24. Be part of the emergency response on UHH site for peri arrest, cardiac arrest situations, and falls with injury.
25. Adhere to Trust and locally agreed policies, guidelines and protocols.
26. Promote the concept of Clinical Governance.
