Main duties of the job
2. The role is for a paramedic/ECP to support the practice's in providing care to acutely unwell patients at home and in the practice as well as providing routine checks for long term conditions within the patients home.
3. The paramedic/ECP will work as part of the team with close collaborative working with other members of the team and support from GPs.
4. To provide holistic care for individuals including assessment, diagnosis, and management.
5. To refer and/or signpost patients in their own home to other allied health professionals.
6. The post holder will use advanced clinical and communication skills to provide education to patients, promote self-care and empower them to make informed choices about their treatment.
7. Care occasionally may need to be provided in other settings as required.

Job responsibilities
The post holder is an experienced paramedic/ECP who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with a range of multi-professional teams to meet the needs of patients, supporting the delivery of policy and procedures.
As a professional and experienced paramedic/ECP, the post holder will undertake/demonstrate:-

Working closely with the site leadership team, the NP will support the delivery of the following:-
