Job summary
This is an exciting opportunity for a diabetic nurse tojoin a friendly, dynamic and forward-thinking practice located in the East ofSouthampton. We are interested in a candidate who is already a fullyfunctioning diabetic nurse.. As a Diabetic Nurse, you will provide care for patients with Diabetes type 1 and 2, providing reviews and foot checks face to face and over the telephone.
The working pattern is negotiable withopportunities for flexible working arrangements.

We are a diverse and friendly team ofGPs, Physician Associate, Clinical Pharmacists, Advanced Nurse Practitioners,Paramedic, Social Prescriber, Nurses and HCAs who work together to provide high-qualitycare for 20,000 patients in the Southampton area.

Duringthe COVID pandemic, we have adapted our service to operate a triage system thathelps us identify and prioritise our sickest patients and those who can safelybe re-directed to self-care services. We also launched an online platformcalled eConsult, where patients can contact us digitally. All our rooms areequipped with video calling and double screen monitors.
We are active in the Bitterne Primary Care Network,with a partner having shared responsibility as a clinical director. Our PCNEducation Environment Lead also helps to ensure staff opportunities to meet anypersonal development goals.

1. GCSE grade A to C in English and Maths
