Job Summary Under the supervision of the IPU Charge Nurse/ Day Services Team Leader and Staff Nurses and in collaboration with the multi-disciplinary team the post holder will actively participate in the delivery of evidence based person-centred care. Main Responsibilities Carry out individual patient care needs, ensuring safe, effective person centred care is carried out with dignity and respect under the supervision of staff nurse. Work collaboratively with all members of the MDT ensuring effective communication and the delivery of high standards of care. Provide comfort and spiritual support for patients and/or their family members on a regular basis Maintain own safety and that of a clean and tidy environment Person Specification Education / Qualifications SVQ 2 in Health Care or equivalent, or evidence of working towards this qualification. (Essential) Knowledge/Experience A proven interest in palliative care. (Essential) Recent experience within a Health care environment as a health care assistant. (Essential) Experience and knowledgeable in nursing care and all activities of daily living. (Essential) Ability/Skills Confident and competent in use of moving and handling equipment. (Essential) Able to document care notes in a variety of forms. (Essential) Effective communication skills. (Essential) Motivated and keen to continue to develop own practice. (Essential) Confidence in own skills/abilities and able to escalate to appropriate staff member if required. (Essential) Personal Qualities Compassionate/empathetic.
