Responsibilities
Assist care managers in developing individualized care plans for clients.
Coordinate and schedule appointments with healthcare providers and specialists.
Support clients and their families in understanding care plans and healthcare options.
Maintain accurate documentation and records related to client care and services.
Monitor client progress and follow up on care plan implementation and outcomes.
Provide administrative support, including answering phones and responding to inquiries.
Participate in team meetings to discuss client care and share updates on client status.
Requirements
High school diploma or equivalent; associate degree in healthcare or related field preferred.
Previous experience in a healthcare or social services setting is desirable.
Strong organizational skills with the ability to manage multiple tasks effectively.
Excellent verbal and written communication skills.
Demonstrated ability to build rapport with clients and their families.
Proficiency in using electronic health records (EHR) and other healthcare management software.
Empathy and a genuine desire to help others in navigating their healthcare decisions.
Hours Per Week: 36.00
Start Time: 08:30
End Time: 17:00
Pay Per Hour: £18.66
Location: Bromley, South East London
