Village Medical Practice in Oldham is looking for a Practice Nurse to join our friendly team as we look to better serve the needs of our patients. You will be supporting patients to be healthy by monitoring long-term conditions, and by focusing on health prevention and screening. You will be working with a team of GPs, a Nursing team, and an excellent admin/reception team that keep the show on the road. There will be professional development opportunities, including cross-practice meetings with other Practice Nurses and opportunities for further training.
Main duties of the job
The post holder will be responsible for the delivery of Practice Nursing services, alongside another Practice Nurse, to the practice population. Supported by the Practice team, they will deliver care and focus on supporting patients to be healthy, monitor long-term conditions, health prevention, and screening activities. They will work collaboratively with the General Practice team to meet the needs of the patients, supporting the delivery of policy and procedures.
Job responsibilities
* Assess, plan, develop, implement, and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
* Implement and evaluate individual treatment plans for patients with a known long-term condition or at risk of a long-term condition.
* Support patients to adopt health promotion strategies that promote patients to live healthy.
* Support and advise patients requesting information relating to family planning needs.
* Support and manage the health needs of patients presenting for cervical cytology consultations.
* Implement and participate in vaccination and immunisation programmes for both adults and children.
* Advise, support and, where appropriate, administer vaccinations for patients travelling abroad.
Person Specification
Qualifications
* Registered First Level Nurse.
* Relevant Nursing degree courses in Chronic Disease Management.
Knowledge
* Knowledge of needs of patients with long-term conditions, accountability of own role and other roles, health promotion strategies, clinical governance issues in primary care, patient group directions, and associated policy.
* Ability to identify determinants of health in the local area, knowledge of public health issues, awareness of local and national health policy, and issues within the wider health economy.
Experience
* Minimum 2 years post-registration experience, management of long-term conditions, involvement in implementing and using protocols and clinical guidelines, and experience of audit.
* Experience within General Practice and nursing experience of running Chronic Disease management clinics e.g., Asthma, Diabetes, etc.
Skills
* Clinical skills; immunisation and vaccination, phlebotomy, examination skills e.g., BP, pulse, BLS.
* Change-management skills and ability to support patients to change their lifestyle. Communication skills, both written and verbal. Ability to communicate difficult messages to patients and families. Negotiation and conflict management skills. IT skills with clinical systems such as EMIS Web.
* Clinical skills; cervical cytology, diabetic foot checks, AED. Experience of teaching and mentorship in a clinical setting.

£38,000 to £41,000 a year depending on experience and qualifications such as prescribing.
