Registered Nurse/Registered Practical Nurse (0.6 FTE, contract)
The Thames Valley Family Health Team is one of the largest family health teams in Ontario. With more than 115 physicians across 19 sites within London, and Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive inter-collaborative primary health care to over 162,000 patients.
The Registered Nurse (RN)/ Registered Practical Nurse (RPN) provides primary care functions and participates in the development, implementation and evaluation of services for the patient care population of Thames Valley Family Health Team (TVFHT). Working in collaboration with a Multidisciplinary Healthcare Team, while providing support to the Most Responsible Provider(s) (NP or Physician), the RN/RPN will work to full scope by sharing expertise and knowledge of health promotion, disease prevention, and chronic disease management across the patient’s lifespan.
DUTIES AND RESPONSIBILITIES:
1. Reviews, screens and prioritizes patient referrals obtained through NP and physician referral, laboratory reports and health records.
2. Conducts consultations with clients to obtain relevant information and assess client health needs.
3. Analyzes the findings of a health assessment and provides navigation support to meet patient care planning.
4. Observes and records health information needed to prepare patients for their appointments.
5. Triage care and respond to patient inquiries to meet expected outcomes.
6. In collaboration with Nurse Practitioners, Physicians and all interdisciplinary health professionals, develops and discusses appropriate individualized care plans with the patient/family caregiver based on best practices.
7. Completes nursing tasks as needed in collaboration with NPs during NP appointments.
8. Provides nursing care and treatment including complementary therapies and/or counselling for identified health-related issues.
9. Documents in patient medical records, refers to and arranges follow-up appointments as necessary.
10. Collaborates in the development of goals and objectives, services and activities, outcome measures and evaluation for individual, group or population program programming.
11. Implements clinical services as authorized by the Most Responsible Provider (NP or MD).
REQUIREMENTS:
1. Licensed as a Registered Nurse or Registered Practical Nurse – in good standing with College of Nurses.
2. Current registration with the College of Nurses of Ontario.
3. Demonstrated ability to work effectively as a member of an interprofessional team, with excellent interpersonal and communication skills.
4. Experience working with an Electronic Medical Record (Accuro preferred) and computer skills.
5. Demonstrated experience in Family Health Team, Community and/or Primary Care Setting.
HOURLY RATE:
Registered Practical Nurse: $27.5306 - $33.57387 (salary dependent on experience)
Registered Nurse: $33.0506 - $40.3056 (salary dependent on experience)
