Job summary
The role of Clinical Practice Specialist Extended Hour service is fundamental to the developing of these innovative new ways of working. The role will support the ongoing development and mobilisation of integrated mental health service offer. The role provides and promotes flexible and responsive access to assessment, treatment and/or onward bridging to community resources and support, in line with evidence-based interventions for people with serious mental illness. The role will help people to focus on achievable goals and access local community resources.
Main duties of the job
The service offer will be for people over 18 years (including carers) who are experiencing mental health difficulties but with a specific focus on: inequality of access by ensuring availability of mixture of in-person face-to-face and remote (telephone, video or online) appointments Monday to Friday 8-8pm. This is especially for those who struggle to access/receive support via the traditional 9-5 operating hours.

Job responsibilities
1.Key Responsibilities
Providefollow up care for service users who struggle to access care and support throughthe traditional 9-5 operating hours as well as offer care and support to otherservice user who requires support within 8-8pm.
Workclosely with colleagues in specialist community mental health services toensure smooth transitions between teams and services and facilitate an easyin, easy out approach to improve access to evidence-based interventions.
Supportservice users through co-produced assessments and evidence-based interventions,including the completion of Dialog+ care plans.
Supportwith processing referrals, triaging and liaising with colleagues and carers inthe provision of holistic support.
Sharein the clinical management of a defined caseload and develop an interface withother services and agencies so that service users are offered focussed andco-ordinated packages of care which are appropriate to their needs.
Alongsidethe Team and Service Manager, take a leadership role in the provision of out ofhours support, supporting with the development of internal processes and the monitoringof the impact of the support provided.
Financial responsibility
To operate within agreed budgets and ensurecompliance with Standing Financial Instructions and other policies and procedures andany audit recommendations.
Responsibilityfor Human Resources
Tomaintain compliance with the Trusts approach to Human Resources, including thedeveloping of training plans for the teams .
Inline with the Trusts polices, take the leading role in the investigation ofserious incidents and complaints .
Responsibilityfor Administration
Toproduce high level reports to the Division as required.
Tomaintain up to date clinical records in a timely and accurate manner in linewith standard operating procedures.
Improveand maintain statistical records of work data within the services to includeown and other accurate records.
Reportto the Team Manager any serious untoward incidents and ensure documentation andrecords are in accordance with Partnership guidelines.
Todemonstrate and awareness of the Partnership policies, procedures, legislationand practice within the guidelines in particular critical incident procedure,child and adult safeguarding, risk assessment.
Responsibilityfor IT and Digital Systems/Services
Ensuresystems are in place to collect social care performance activity management data as required.
2.Clinical Responsibilities
Towork in collaboration with clinical leads and all professionals in the MDT andclinical leads.
Providesa brief clinical risk and mental health assessment, formulation and short-termcontingency planning for people presenting in mental health needs or support.
Providecare or support via telephone/Video or a face-to-face assessments.
Workclosely with other mental health services and agencies; such as the police,ambulance, local authority and third sector.
Usinga range of brief solution-focused interventions, to empower people, carers andtheir families in decision making; diverting to a range of health and socialcare resources for support.
Communicatewith service users and callers at their level of understanding, culture andbackground. Always remain courteous and professional.
Provideskilled and detailed triage, full assessment and intervention to clients forall follow up or new referrals to the team. In the event of support during mentalhealth crisis, re directing to the most appropriate services.
Offersupport and advise to carers in prevention and management of crisis situation.
Provideclinical advice, education, support and training to team colleagues, CMHTs,primary healthcare teams, other professionals, service users and carers on themanagement of mental health problems.
Delivercare that is evidence based, reflecting current best practice and research.
Tomaintain safe and acceptable standards of practice in accordance with Trustpolicies and procedures.
Participatein the development, mentorship and clinical supervision of staff and students.
Practicein a way that minimises dependency and promotes recovery.
Demonstratea sound understanding of the legal and ethical issues in the provision ofmental health support.
Inparticular, demonstrating sound understanding of the Mental Capacity Act (2005)and the Mental Health Act (2007) and its use in a non-mental health setting.
Provideclinical advice and support to other professionals about risk management andmental health via the professionals' line.
Tokeep professionally updated with ongoing developments in mental health thoughan evidence-based approach to delivering high quality mental health services byevaluating, updating, imitating and/or participating in training, audit andresearch.
Thepost holder will be expected to adopt a flexible attitude to take account ofthe changing needs of the service as dictated by the service user andorganisational lead.
Continuethe promotion of a culture of evidence-based practice and support specialistpractitioners in their role
Reducingand managing risks to service users through collaborative working together withservice users/carers in their treatments and adherence to their care-plans.
5. Operational Responsibilities
Deputisein the absence of the Team Manager
Thepost holder will be required to undertake any other duties and responsibilitieswithin the framework of the post as determined by the Managing Director.
This post is subject to an annual review of performance to agreedtargets. Targets will be agreed with theManaging Director and the post-holder and will be subject to regular review ofprogress.
