Job summary Rehabilitation Assistant - Harrow Therapy Service Band 3 CLCH Harrow Adult Community Services Therapy Services are now welcoming applications from an experienced Rehabilitation Assistant This is a great opportunity for someone who is conscientious and adaptable. The successful candidate will have excellent communication skills and will be able to demonstrate an adaptable approach whilst meeting guidelines associated with Therapy. They must be confident working as a lone worker, with the support of the team. The Therapy model includes: Discharge to Assess, Short Term Rehab and Intermediate Care supporting Therapy in Bedded Units The post holder will therefore be required to support clinicians in achieving Therapy goals in the community i.e. visit patients in their own homes or Bedded Units to carry out prescribed treatment and assess for and provide appropriate O/T equipment or mobility aids directly or by prescription. The post holder will work alone and will be required to use a degree of initiative while working within the parameters of the team's policies and procedures. The service provides multidisciplinary therapy (OT and Physiotherapy) and support to help patients engage in a rehabilitation or long-term management programme to regain or retain their independence. You must be a car owner and have a valid UK driving license as this role involves working with patients in their own home, covering West Hertfordshire. Main duties of the job To carry out pre-defined rehabilitation programmes with patients in their own homes, day centres, the local environment, including outdoors and on public transport, and within the therapy centre as directed by a qualified therapist for a range of patients, including those with complex presentations. To report to an allocated supervisor about treatments undertaken, progress made, outcomes of intervention and to discuss when modification of rehabilitation programmes may be necessary as part of a structured clinical supervision system. To ensure all aspects of your work are of a high clinical standard and be accountable for this to the team lead, through the qualified staff. To observe any progress, deterioration or other significant changes in a patient's condition or performance and to report these to the relevant therapist involved in the client's care. To participate in the delivery of patient education programmes. To undertake the measurement and evaluation of your intervention through the use of treatment plans, goals, outcome measures and clinical supervision with a qualified therapist in order to progress treatment programmes and ensure clinical effectiveness.
