Select support partnerships are GROWING! We are looking for a experienced Senior support worker to join our expanding services, working between Blackburn and Preston.
The ideal candidate will have:
* UK Driving license
* 1 year + leadership/senior experience
* Willingness to learn and develop on the job.
Responsibilities:
To take a lead role in supporting Service Users in their own homes and in the community through person centred approaches in line with their individual support plan
To provide flexible and supportive services to maximise Service User independence
Work within the context of Select Support Partnerships policies and procedures
Main Responsibilities and Duties General
Work in partnership with Service Users, their families and representatives to implement all aspects of their support plans.
Encourage Service Users to have a proactive approach to their lives and to maximise their inclusion and participation in the community according to their needs, wishes and preferences.
Promote equality and diversity within the service and the wider community.
Maintain confidentiality at all times.
Support Service Users to access a range of opportunities and experiences either from their own home or the wider community.
To support Service Users to monitor their health and wellbeing.
Attend team meetings and supervision meetings and contribute to the promotion of good practice across the service.
Undertake all statutory training and comply with legislation requirements.
Support Service User’s to reach their full potential and to assist them to meet their goals.
Uphold and maintain professional boundaries.
Facilitate communication and help Service Users to express their needs, feelings and wishes.
Service User Support
Participate in the full range of support duties including:
Supporting Service Users with all aspects of their agreed support plan.
Where required and appropriate, support Service Users with all aspects of their personal care needs and ensuring the individuals rights to privacy and dignity are respected.
Support Service Users to manage their medication, including administration where appropriate and to support individuals to have an understanding of why they take their medication.
Support Service Users to develop skills to enable them to manage their daily lives and to promote their independence.
Supporting and encouraging all domestic duties (washing, cleaning, cooking etc.)
Support Service Users to manage their finances as required within their support plan.
Support Service Users to access paid or unpaid employment / voluntary work, education and leisure opportunities.
Support Service Users to deal efficiently with all post and correspondence.
Support and accompany Service Users on holidays, outings, trip, visits etc. as required.
Facilitate and support meaningful day activities including attending college and work placements etc.
Support and promote the development and maintenance of independent living skills.
To help ensure that the Service Users health needs are met including support liaising with health professionals, visiting health services and ensuring the implementation of appropriate medication procedures.
Assist Service Users to develop and maintain relationships with family and friends.
