The post-holder will work across the CYGNET PCN demographic as an autonomous, accountable advanced clinical practitioner to assess, consult and treat patients in residential/care homes and in primary care settings. A variety of consultation skills, both face to face and remote, will be key to providing and supporting this cohort of our population. The post-holder will be suitably qualified with a valid registration with HCPC or NMC. An independent prescribing qualification is desirable. Patients will be triaged, assessed, and appropriately signposted in accordance with agreed rotas and systems. These will typically be patients presenting with a wide range of acute illness, urgent, same day requests for treatment and chronic disease management. You will be providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
