An exciting opportunity has arisen for a part-time Band 7 Advanced Nurse Practitioner to join our Solihull Neighbourhood Mental Health Team in the Solihull area.
Our community mental health services are well-established teams, and as part of the BSOL community transformation programme, we are looking to increase our primary care teams with these roles. This is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care. The development of our Primary Care mental health team is an essential service which we are in the process of evolving in line with the NHS Long Term Plan.
Main duties of the job
We are looking for an enthusiastic and dynamic individual who is able to work within a multi-disciplinary team, working alongside other professionals and striving for the best outcomes for individuals within the local community that the team serves.
You will be responsible for:
1. Providing mental health triage, assessment, and outcomes-based intervention for service users including nurse prescribing within the Primary Care Mental Health Team.
2. Identifying need and the best course of advice, support, or intervention.
3. Promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.
4. Ensuring robust communication across FTB/BSMHFT and primary care in accordance with each individual service user's needs by providing support and advice to all colleagues with the PCN as required, including participation within the local neighbourhood MDTs.
