The successful candidate willbe requiredto provide high quality, responsive services working as an integral member of the intermediate care team and wider Integrated Community team. You will work closely with social services and voluntaryorganizationfor adults with a wide range of physical impairments and mental health conditions.
The successful candidate will work autonomously under the guidance of the team manager (asrequired) having a clinical responsibility for a designated caseload as well as supporting the clinical leadership of the team and management responsibilities.
Reading Intermediate Care is a team of physiotherapists, occupational therapists,senior therapy assistants,triagecoordinators andadministrators.
This supportivemultidisciplinary teamwork with patients across Reading in close communication with social services, voluntaryagencies,and GPs. This postinvolves7 days working within a shift patternwithin the intermediatecareservice.
• To post holder tomangethetimelyassessment and management of referrals with multiply co-morbidities and complex rehabilitation needs.
• Support all intermediate care pathways to promote thetimelyassessment and management of referrals with multiply co-morbidities and complex rehabilitation needs.
• To support more junior members of the intermediate care team with clinical development and caseload management.
• Toact as lead totriage the referralsfor intermediate care services
• Provision of equipment to promote independence and safety.
• Hold a caseload and work autonomously
• Work flexibly to ensure urgent referrals are adequately managed
• Supporting colleagues in the delivery of compassionate,safeand structured care
• Provide guidance andappropriate delegationwithin the team
• To support the teammangerandparticipatein service development and quality improvement.
• The post holder will hold responsibility for the provision of education and clinical physiotherapy advice to staff/studentsin Intermediate Care.
