The Relief Team District Nurses has an exciting opportunity to recruit to a permanent band 5 position, working 37.5 hours weekly over 7 days.
We are looking for an enthusiastic and highly motivated band 5 Community Nurse who will be happy to work in an evolving district nursing service. The successful applicant will be committed to working closely with health and social care colleagues to provide high quality care to patients with a wide range of medical conditions in the community setting.
The role requires excellent communication skills as you will be undertaking complex nursing assessments and developing and delivering programs of care for patients and carers. The district nurses work a 24hr service and there is an expectation that you may be required to support this to ensure service delivery which will include working regular evening shifts and night shifts.
You will need to work collaboratively with other professionals, voluntary sector and informal carers to promote excellent care to patients in their own homes and community setting.
You will be joining an enthusiastic and motivated team who are friendly and approachable. We are very patient focused and staff development is high on our agenda. As part of our team we want to develop all staff to the highest standard.
Main duties of the job
Anyone newly appointed to the trust will be subject to the completion of a satisfactory 6 month probationary period (this includes bank staff). This will give the new staff member an opportunity to become familiar with the role and working environment.

* Recent wound care experience
* Broad knowledge base of acute and chronic conditions.
* Understanding of scope of professional practice.
* Demonstrable experience of using initiative and effective prioritisation

* Community nursing experience
Training
* Evidence of clinical/professional development.
* Nurse Prescribing
* Wound care qualification
* Post registration qualification in Acute/Chronic Disease Management

Teaching
