As a Social Worker within the Pathways Team, you will manage a caseload of young people, conducting assessments and developing Pathway Plans to support their transition to independent living.

* • Identify young people's needs, aspirations, and support requirements in areas such as education, employment, housing, and emotional well-being.
* • Provide ongoing support and guidance to care leavers, helping them access appropriate services and opportunities.
* • Collaborate with internal and external agencies to ensure a coordinated package of support.
* • Advocate for young people's rights and ensure their voices are central to the decision-making process.
Key qualifications include:
* • Qualified Social Worker registered with Social Work England.
* • Experience in working with young people, particularly in a leaving care or transition-focused role.
* • Strong knowledge of relevant legislation, including the Children (Leaving Care) Act.
