About the Role
Social Care Reablement is a specialist Homecare team that work across the city to provide support to citizens in their own homes. The focus of the work done by the teams is to help citizens reintegrate back into their previous lives, whether that is socially, physically or emotionally.
The teams encourage independence and choice for citizens, and control over the way their care is provided. This involves not only providing personalised care as directed in the citizen’s care plan, but also encouraging and enhancing the wellbeing of citizens through promotion of independence, early intervention, community support and friendships

* Knowledge and understanding of the rights, needs and feelings of vulnerable adults from diverse social and cultural backgrounds;
