Description of the duties
Clinical
1. To assess, plan, implement and evaluate specialist interventions for people with learning disabilities that are person centered and in consultation with the service user as part of their overall care plan. This will include this specialist therapies and behavioural support, depending on the role of the clinician (for example, physiotherapy, OT, SALT or LD Nursing).
2. Assess clients understanding of treatment proposals and their capacity to give informed consent and working within the legal framework when the client lacks mental capacity to make the required decision.
3. To have the ability to assess risk and develop positive risk management plans, including crisis and contingency plans, for service users as part of a multi-disciplinary team.
4. Apply specialist knowledge and skills to meet the needs of those with complex health needs, including challenging behaviour.
5. To take a lead role in the management of the general health and well-being of service users, acting as Care Co-Ordinator as required.
6. To safely discharge the service user at the end of your episode of care.
7. To keep patient clinical records and statistics as per trust and professional guidelines with the use of information technology – ensuring timely, contemporaneous and accurate records.
Leadership
1. To provide leadership and supervision to designated junior staff ensuring that they receive advice and support and that the delegation of tasks is appropriate to their ability and grade.
2. Contribute to the planning, organization and maintenance and leadership of professional networks and interest groups to maintain and develop professional practice.
3. Participate in training of self and others as relevant to the post, including mentoring and supervising students on placement.
4. To care for own health and safety as well as that of service users and colleagues, adhering to arrangement made both locally and by the organization.
5. To contribute to the evaluation of work and current practice by undertaking small projects to support LD service development i.e. audit and research.
Most challenging part of this role
Working within a busy community team with people who have complex health needs can be challenging. There will be competing demands and it may sometimes be difficult to prioritise work across the key result areas. It is essential that all team members work together, sharing knowledge and skills and supporting each other to ensure high quality care for our service users.

Desirable criteria
* Community Caseload Management
* Learning disability experience
* Experience of supervising staff.
