This is an exciting opportunity, to be part of a specialist Outreach service for clients with dementia inresidential/nursing care providing specialist nursing/healthcare advice, treatment andteaching/education as appropriate under the guidance of professionally registered teammembers. To contribute to assessment and interventions carried out by the dementia outreach. to engage with the Enhanced Health in Care Homes Service (EHCH) andwith GPs in the local area. Part of a multi-disciplinary Outreach service which works with both Independent Sector and Statutory Sector providers of Residential Care to Older People with dementia. To increase the capacity, confidence, competence and willingness of those organisations to provide the right level of care to this vulnerable client group and by doing so to increase standards of care, improve quality of life, increase independence and choice. You will be supported through training and supervision. As this role is community based covering the Broxtowe area, afull UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010. Please note that this post does not meet the pay level required for a Skilled worker visa. Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.
