Are you a Registered Mental Health Nurse with a passion for supporting people as they reach the end of their life? Are you a dynamic and forward-thinking, person-centred mental health practitioner?
We have an exciting, new opportunity to shape and develop services for the people St Christopher’s Hospice support who are living with severe mental health illness. You will be working alongside our strong psychospiritual team and psychiatrists.
This is a new role at St Christopher’s Hospice, based in South East London, to offer people the skills of a Mental Health Clinical Nurse Specialist in relation to their mental health needs, as well as their emotional, spiritual, social, and physical care needs. The role is designed to develop services that ensure we are delivering high-quality care to those with enduring and acute mental health conditions, aligning with providers across the local health economies, and inspiring good practice and innovation.
The role will include:
* Developing educational packages for St Christopher’s staff to empower them to support patients presenting with enduring and acute mental health conditions as a comorbidity.
* Working with our professional learning team to develop the skills of a workforce wider than St Christopher’s.
* Working directly with our patients.
* Offering appropriate advice, support and education to the primary health care team and hospital staff, as part of an interdisciplinary team around care for people presenting with enduring mental health conditions and a terminal illness.
* Taking an interdisciplinary and holistic approach to care.
* Participating in quality initiatives to improve organisational practice.
* Continuously reflecting on and sharing learning from this dynamic role.
The role will be based at both the Sydenham and Bromley sites working closely with the In-Patient Unit and also the Community Palliative Care Team who provide care and support to people who live in the London boroughs of Bromley, Lambeth, Southwark, Lewisham, and Croydon. At any one time, our team is supporting around 1900 people. We work seven days a week, and our clinicians work between the hours of 7.30am - 9pm.
Every day our team of clinicians work across the whole organisation to:
* Take new referrals, and signpost health and social care providers to improve access to end of life care.
* Triage calls from patients, their families, and other health and social care professionals to provide telephone advice and support.
* Contact people by telephone or video consultation; see people in their own homes, or in our outpatient clinics, to assess and plan care and support interventions.
* Undertake urgent home visits when necessary.
Our aim is to support people to be cared for in their own homes for as long as possible. We work with the in-patient unit at St Christopher’s to support people when they need extra care that cannot be provided at home. Our team also actively participates in, and contributes towards education and learning with our colleagues in St Christopher’s CARE (Centre for Awareness and Response to End of Life).
If this new opportunity has excited you, please do have an informal discussion with Amanda Mayo, Care Director on 07941 506562 or via email a.mayo@stchristophers.org.uk.
* Closing Date: 16th March 2025 at midnight
* Provisional interviews will be held between 26th March 2025
