Main duties of the job You will be a key member of the Porth Preseli Integrated Hub which provides a central point for the referral and provision of integrated community services, facilitating safe and timely care provision for our population. Our Coordinators are expected to be kind and responsive, professional and informative and contribute to the quality of our services by demonstrating the behaviours detailed in our Values. You will receive queries via telephone and email conversations and will resolve issues using your expertise of pathways and processes. To be successful in this role you will need excellent time management skills, must be able to work on your own initiative, under pressure and be able to prioritise your own workload with minimal supervision.The role is diverse, including taking referrals, being part of MDT discussion, liaising and arranging services. You will be working with a number of different professionals in a busy, supportive environment. Some knowledge of health and social care services would be beneficial as you will be dealing with diverse referrals and pathways, gathering information and coordinating responses.
