The Advanced Nurse Practitioner will work autonomously providing high quality and timely patient centered care. The post-holder will be a non-medical prescriber, but not essential, who is motivated, proactive, committed and has excellent communication skills.
The post-holder will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills., The successful candidate will be supporting the Shropshire Rural Alliance PCN to achieve the successful delivery of our Direct Enhanced Service (DES) specification, specifically working with our Care Homes, delivering Chronic Disease clinics, and supporting with our Extended Access service. The post-holder will be supporting the PCN practices by working closely with the Clinical Director and Management Team.
