Duties & Responsibilities:
* Perform diabetes self-management education (DSME) and nutrition assessment.
* Provide comprehensive analysis of assessment results to clinic endocrinologists and referring providers.
* Support and assist in coordinating follow-up care for patients with diabetes and complex medical needs.
* Perform the duties of a Dietitian Nutritionist; assess nutritional needs, diet restrictions, and current health plans; develop and implement dietary-care plans; and provide nutritional counseling.
* Advise patients and their families on nutritional principles, dietary plans, diet modifications, and food selection and preparation.
* Counsel individuals and groups on basic rules of good nutrition, healthy eating habits, and nutrition monitoring.
* Consult with physicians and health care personnel to determine the patient’s nutritional needs and diet restrictions.
* Assist with the care of patients with pre-diabetes and diabetes in an outpatient and telehealth setting.
* Evaluate diabetes self-management skills in patients.
* Monitor outcomes of DSME completion and present data for divisional and institutional review.
* Coordinate and lead diabetes education classes.
* Review available medical reports and assessments.
* Utilize education guidelines to promote optimal management of diabetes.
* Complete patient training education related to diabetes technologies.
* Coordinate insurance approvals and device delivery.
* Perform other related duties and participate in special projects as assigned.
Minimum Qualifications Required:
* Bachelor’s Degree in public, mental, and/or medical health education, social work, psychology, student personnel/affairs, nursing, education or communications.
* Two (2) years of experience conducting professional presentations, seminars or closely related programs and/or providing health education.
Note: An applicant with a Master’s degree in an area of health education qualifies with one year of professional communications and/or health education experience.
Preferred Qualifications:
* Possession of current Registered Dietitian licensure from the Illinois Department of Financial and Professional Regulations.
* Certified Diabetes Care and Education Specialist (CDCES).
