Job overview
The Acquired Brain Injury Team offers an exciting opportunity for a Team Lead & Case Manager to join our community specialist team and support the delivery of neurorehabilitation to patients living with an acquired brain injury in the Community. We are looking for a suitably qualified, experienced, and registered professional from a field such as social work, nursing, occupational therapy, physiotherapy, or speech and language therapy.
The team has an administrative base in Carlisle and Kendal and covers the whole of Cumbria, so the successful candidate must be prepared to travel as part of their job role.
As a Team Lead, the post holder will provide clear direction, support, and guidance to the Acquired Brain Injury Service by adhering to and implementing service plans, Trust Policies, and Standard Operating Procedures.
As a Case Manager, the post holder will lead on new referrals and assessments of patients into the service, working collaboratively with the interdisciplinary team to design and implement specialist rehabilitation plans.
Main duties of the job
* Provide specialist assessment and rehabilitative packages of care to patients with brain injury aimed at reaching the clients' full potential.
* Work with other health, social care, and voluntary agencies regarding the repatriation of patients.
* Provide direct care for complex cases, working with patients and their families.
* Consult with teams to support the case management of patients' care and treatment.
* Supervise other professionals, including Occupational Therapists, Physiotherapists, Speech and Language Therapists, and Rehabilitation Assistants.
* Contribute to the continued professional development of other Allied Health Professionals within the service.
* Provide group interventions alongside other Allied Health Professionals.
* Coordinate new referrals into the service, ensuring assessments are carried out in a timely manner.
* Monitor and evaluate the goal-oriented process for all patients, collaborating with the team and the patient.

Qualifications
* Professional qualification recognised by HPC, UKCC, General Social Care Council, or NMC.
* Evidence of postgraduate training/experience in neurorehabilitation.
Experience
* Experience of working with a variety of patient groups across the life course and presenting with a full range of clinical severity.
* Experience of teaching, training, and/or professional and clinical supervision.
* Experience of working within a multicultural framework.
Skills, Knowledge, and Aptitudes
* Demonstrate clinical knowledge in neurorehabilitation.
* Ability to plan and organise in a manner consistent with the service remit.
* Well-developed communication skills for conveying highly technical and clinically sensitive information.
