The ward is nurse led which will give you high levels of autonomy and satisfaction in delivering exceptional care to patients with sub-acute needs, supporting rehabilitation, undertaking complex discharge planning and increasing patient independence and quality of life. Our patient cohort is increasingly complex due to the need to facilitate transfer from acute trusts as soon as individuals are medically fit for discharge and to prevent admission from home to the secondary care sector.,
* Assessing, planning, implementing and evaluating patient care in a nurse led environment.
* Assisting with and contributing to the management and organisation of the ward.
* Delivery of high-quality care.
* Participating in education, development and supervision of other staff members and students.
* Working collaboratively and co-operatively with others to meet the needs of patients and their families.
